The Health Insurance Portability and Accountability Act of 1996 required all covered entities (e.g., Providers, clearinghouses, and large health plans) begin using National Provider Identifiers (NPIs) on standard healthcare transactions by May 23, 2007. The purpose of the NPI is to uniquely identify a healthcare Provider in standard transactions such as healthcare claims and eliminate the need for legacy Provider numbers.

How to Apply for an NPI

A Provider may apply for an NPI through a web-based application or may send a paper application form. The web address is https://nppes.cms.hhs.gov. A Provider may also call the NPI Enumerator at 1.800.465.3203 or TTY 1.-800.692.2326 for any questions regarding the application. There is no cost to apply for an NPI.
