How to Become a Participating Provider

Application Process

  1. Complete the applicable profile form below to request participation in Community’s programs.
  2. If you are currently part of an IPA, please contact them to determine if they currently have an agreement in place with Community.

Credentialing & Recredentialing Process

As a Medicaid managed care organization, Community Health Choice must utilize the Texas Association of Health Plans’ (TAHP’s) contracted Credentialing Verification Organization (CVO) as part of its credentialing and recredentialing process regardless of membership in the TAHP. The CVO is responsible for receiving completed applications, attestations and primary source verification documents.  At least once every three years, Community Health Choice must review and approve the credentials of all participating licensed and unlicensed Providers who participate in the network.

Upon receipt of your signed contract and “Completed” Credentialing file from Verisyi (CVO), the credentialing process can take up to 90 days.

Important: Please ensure that your credentials and information are current to avoid any delays in the credentialing or recredentialing process.

Attendant Compensation Rate Enhancement Program (ACEP)

Attendant Compensation Rate Enhancement Program (ACEP)  

The Attendant Compensation Rate Enhancement program is a voluntary program for HHSC contracted providers delivering community Care Services. Participating providers will receive increased funding to incentivize increased compensation, including increases in wages  and benefits, for attendants.  

Enhanced Program enrollment categories: 

  1. Individualized skills and socialization services (formerly day habilitation) 
  2. Non-Individualized skills and socialization services (formerly non-day habilitation)  
  3. Residential Services 

Note: Annual enrollment period is from July 1, 2024 – August 15, 2024. This year’s rate enhancement will be effective September 1, 2024. 

Community will tie the attendant compensation rate enhancement rate add-ons to the appropriate fee schedule per participation levels.
