Medicare Dual Eligible Special Needs Plans (D-SNPs) are Medicare Advantage plans for people with both Medicare and Medicaid. The Community Health Choice (HMO D-SNP) combines all the benefits of Original Medicare (Parts A and B) with prescription drug coverage (Part D), providing extra coverage to help Members better manage their unique healthcare needs.
D-SNPs enroll individuals who are entitled to both Medicare (title XVIII) and medical assistance from a state plan under Medicaid (title XIX). States cover some Medicare costs, depending on the state and the individual’s eligibility.
Community provides its D-SNP Members with access to comprehensive medical care, including services such as hospital care, surgery, X-rays, physical/speech/occupational therapies, prescription drugs, emergency services, durable medical equipment, transplants, routine health checkups, and preventive screenings.
