Disaster Declaration – Effective date

Use this section ONLY if a disaster declaration is issued.


If your county was affected by Crisis, Members may be able to get early refills of your prescription drugs, replace lost or damaged medical equipment or be able to receive services out of network.

Provider Portal: If you can get on the Internet, log in to your Provider Portal to get in touch with us via e-mail.  You can also:

  • Check Member eligibility and benefits
  • Check claim status
  • Retrieve EOPs
  • Submit and check status of authorization
  • Obtain your member panel

Prescriptions: Call Community Health Choice.

Behavioral Health: If you are a Member or you are caring for a Member who is experiencing a life-threatening mental health emergency, please call 9-1-1 or go to your nearest emergency room for help. 

Community also provides 24 hours a day, 7 days a week support through our Behavioral Health Crisis Line. These numbers are also on the back of your ID card.
